We are always happy to discuss your retirement plans, as well as guide you through the required forms and application process. The forms include:
- Western Home Communities Application for Admission/Confidential Financial Statement
- Medical Status Certification (to be completed by your physician)
A one-time entrance fee is payable at the time of the decision to move in. An assessment visit in your home or the hospital will be scheduled. Following this visit and the submission of your application, a decision will be made regarding the appropriate level of care at Western Home Communities.
